The success of the health promotion/disease management professional in managing the health of the high risk patient is one of the most critical roles in healthcare. High cost and high volume Diagnosis Related Groups (DRGs) such as diabetes, stroke, pneumonia, asthma, COPD, chest pain, angina pectoris, heart failure and major joint replacement cost the health-care industry billions of dollars each year.

According to the Center for Disease Control the medical costs for chronic diseases (most of which are caused by high-risk behaviors such as smoking, drug or alcohol abuse, or obesity) account for 60% of the nation’s $1 trillion cost for medical care. Since the introduction of DRGs in the early 1980’s, the health care industry evolved into the current business practice of Managed Care using a capitation model of payment.

In an effort to reduce the expense of high cost and high volume DRGs the healthcare industry has historically used traditional didactic educational modalities to teach self efficacy to a high cost DRG population as a means of population management. The knowledge based, educational interventions have proven to be insufficient in producing the sustained behavior change sought in the management of high cost and high volume DRGs.
